Chicken Soup LLC: Basic Service

• $200—includes visit to your door after 5pm, initial evaluation of your child
• $40—fee for any additional diagnostic testing  ( pulse oximeter, strep/flu/urine)
• $40--Onsite treatments such as nebulizer therapy, steroids, zofran
• $50—per child add-on for other children in the home that you would like evaluated
• $275—flat rate for laceration repair

Chicken Soup LLC: Membership Service

• $1000 includes 6 visits that are good for 12 months
• Any additional diagnostic test/treatment is included at no extra charge
• No additional add-on fee for other children in the home that you would like evaluated
• After 12 months, if you choose to renew your membership, each visit not used can be rolled over to the next year or $100 credit per unused visit, can be applied to the membership fee
• Memberships are nonrefundable
